J. B is a 45- year old male taking sildenafil and tamsulosin. The patient’s wife confides that her husband has been taking sildenafil and tamsulosin but she isn’t sure why they were prescribed.
Why are these medications prescribed?

Ans) J.B might have BPH ( Benign Prostatic Hyperplasia ) as Sildenafil & Tamsulosin are prescribed for BPH.
Sildenafil is prescribed for this patient as it enchances vasodilation effect of nitric oxide in the corpus cavernosus of the penis, thus sustaining an erection, produces a reduction in the pulmonary to systemic vascular resistance ratio.
Uses: Erectile dysfunction, pulmonary arterial hypertension
Side effect/Adverse effects:
Body as a whole: face edema, photosensitivity, shock, asthenia, pain, chills, fa;, allergic reaction, arthritis, myalgia.
CV: MI, cardiac arrest, sudden cardiac arrest
Respiratory: Nasal congestion, asthma, dyspnea, laryngitis, pharyngitis, sinusitis, bronchitis, cough
Skin: Exfoliative dermatitis
Hematologic: leukopenia
Contraindication: Hypersensitivity to Sildenafil
Nursing Implication:
• Assessment and drug effects: monitor carefully vital signs and immediately report S&S of cardiac distress.
• Patient and family education: Do not take sildenafil within 4 hours of taking doxazosin, prazosin, terazosin or tamsulosin.
Tamsulosin is also prescribed for this patient as it blocks alpha receptors in prostate and bladder creating relaxation and improved urinary flow.
Side effects/ Adverse effects:
- Enlarged prostate, erectile dysfunction, headache, abnormal erection, rhinitis, headache, abnormal erection, rhinitis, orthostatic hypotension, dizziness, ejaculatory dysfunction, arrhythmias
Contraindication: Drug allergy
Nursing Care:
Have patient take medication at bedtime due to orthostatic
hypotension.
Report dizziness, vertigo or fainting to physician
Precautions:
- vasodilators that can lower blood pressure
Contrainteractions:
other ABs, CCBs, drugs that induce/inhibit hepatic enzymes
