4a) Briefly describe what is involved in the processing information phase. As a nurse, what is the purpose of processing information?

The processing information is the third phase of CRC.It is considered as the critical and most important phase .The processing information phase include various steps they are;

* Interpret - it involve analyzing the collected data to better understand the signs and symptoms and analyze what is normal and what is abnormal .

* Discriminate -here distinguishing the relevant information from the irrelevant information ,identifying the consistencies , identify what is more important and recognize the gaps in the collected information .

* Relate - identify the relation between the information inorder to identify the relationship between them .

* Infer - develop an opinion which is logical by subjective and the objective data .

* Match - Just as like an expert work match the present situation with the past or the recent patient with the past patient .

* Predict - predict the outcome just as like the expert thought .

Processing information is considered as the core factor in the field of Nursing .processing the information helps the nurse to interpret ,discriminate, relate , infer , match ,and predict which are the key factors for providing the effective patient care .knowledge about processing information helps to document the information adequate ,made available on time and make it up to date and valid ,thus helps in taking good decision regarding the patient care .

While processing the information the nurse first interpret the information .when interpret the nurse can compare want is normal and what is abnormal .example - nurse can understand the client BP is low in an usual hypertensive client .

Descriminating the information helps to act critically by identifying the most important thing .this step is very important in case of emergency .for example - the nurse will not too worried about the little temperature rise and give more concer to the BP and pulse .

Infer helps in reaching a suitable opinion ,eg - his drop in BP is due to blood loss .reaching to the suitable opinion helps to act accordingly

Match helps to identify the relationship ,so the nurse can take appropriate measures to improve care ; eg - processing the client present history with the past helps to know the client get pruritis with the specific drug .

Predict helps to think as like an expert and able to take effective measure to improve the client care .
