You are an exercise physiologist at a hospital-based wellness facility. A client, Drew, recently joins your facility. Drew is a 47-yr-old male. He currently weighs 315 lb. and is 5 ft 11 in tall. His waist circumference is currently 127 cm. Due to his waist girth and weight; body composition estimation was not possible via skinfolds. He currently does no regular physical activity. At his initial consultation with you, you measured his resting heart rate (HRrest) at 58 bpm, and his resting blood pressure (BP) was 138/72 mm Hg. At his most recent doc- tor visit, his fasting blood measures were the following: total cholesterol= 192 mg/dL, high-density lipoprotein cholesterol (HDL-C)= 41 mg/dL, low-density lipoprotein (LDL)= 117 mg/dL, triglycerides= 169 mg/dL, and glucose= 137 mg/dL. On his health history questionnaire, he states that he has previously been diagnosed with high cholesterol, high BP, and diabetes mellitus. He is currently on several medications including Lipitor (statin for cholesterol), atenolol (beta-blocker for BP), and metformin (biguanide for glucose control). He reports that his father previously was diagnosed with diabetes in his 40s and had a nonfatal heart attack at age 52 yr. He also reports that no symptoms suggestive of ischemia.
During your initial consultation with Drew, he discussed his desire to improve his overall health and to try to prevent a premature myocardial infarction (MI). He realizes that he is not in good health and not physically active. He wishes to make the necessary changes to lose weight, to improve his risk factor profile, and hopefully not go down the same path as his father

1. You are a nurse admitting a patient to the hospital from the emergency department (ED) with shortness of breath and recent weight loss. After receiving a report from the ED nurse, you ready the patient’s room according to unit specifications and collect the necessary equipment and forms. When the patient arrives, she is using oxygen via a nasal cannula and seems to be comfortable. As you begin your admission activities and paperwork, you note that her shortness of breath slightly increases as she answers your questions. Accompanying the patient is her daughter, who comments, “This is the fourth time she’s been admitted to this hospital in the past year.” The patient and her daughter demonstrate a close, loving relationship. The daughter not only encourages her mother, but also sets boundaries regarding her mother’s anxiety.

Case Study: Delegation
It is the beginning of the evening shift on a step-down unit. Your team for the evening includes you (the RN), another RN, one LVN, and one UAP. Here are the patients on the unit:
Room 1A: 50-year-old female with ovarian cancer. Had an argument with her husband and is asking that all visitors be restricted.
Room 1B: 88-year-old female with dehydration. Is now confused, and a urine sample is needed to rule out a urinary tract infection (UTI).
Room 2A: 44-year-old male admitted with a nonhealing abdominal wound after gastric bypass. The wound has a negative pressure wound therapy system that needs to be changed this shift.
Room 2B: 78-year-old male with bacterial endocarditis who will be returning to the nursing home to continue his IV antibiotics. Needs transfer form completed, assistance with dinner, and a bath.
Room 3A: 35-year-old male going for an IV contrast CT. Received call from radiology that transportation is on its way to pick up the patient; patient does not have any IV access.
Room 3B: 18-year-old male admitted for 23-hour observation for mild concussion after falling at an ice rink. Patient is ready for teaching prior to discharge.
Room 4A: 22-year-old female with SIADH on fluid restriction. Current fluid restriction is 200 mL for evening shift and 100 mL for night shift. Family has brought in multiple food items as well as her favorite sodas to drink.
Room 4B: 25-year-old female with PID. Needs to receive IV antibiotics, and a weight is required to prepare the proper dose.

Question 2: What determination do you need to make before assigning care of the patient in Room 2A?
Question 3: Which tasks must be completed by you and the other RN? List in order of priority with rationale for each room.
question 4: Which tasks may be delegated to the LVN/LPN? List in order of priority and provide your rationale for each room.
Question 5: Which tasks may be delegated to the UAP? List in order of priority and provide your rationale for each room.
Question 6: What specific feedback does the RN need from the LVN and UAP regarding their assigned tasks?
Question 7: What care delivery model is described in this scenario?
Question 8: The UAP informs you that the patient in 1B has a fever of 102° F. After performing an assessment, you decide to call the health care provider. How would you use SBAR to communicate with the provider?

Create a conceptual map using the case scenarios as guide. Conceptual map should include pathophysiology, medical diagnosis, signs and symptoms, and risk factors, if any. Nursing diagnosis, nursing interventions, medical management (medication and procedures), expected outcomes
A 37-year old man with chronic renal failure who was secondary to chronic glomerulonephritis had been on peritoneal dialysis for approximately 6 months without any episode of peritonitis. In December 2019, he was admitted to the hospital for fever, vomiting, abdominal pain, diarrhea, and cloudy dialysate several hours after eating stinky tofu. The peritoneal effluent culture yielded Aeromonas Sobria. The patient was given levofloxacin for 10 days. The patient symptoms such as diarrhea, abdominal pain were relieved and the cloudy effluent turned to be clear. Unfortunately, peritoneal dialysis catheter was blocked because of fibrin clot formation in the setting of inflammation, and finally it was removed.
