A patient is admitted to the medical floor in a health care facility. She is confused and will not stay in her bed. The patient is placed in a room at the end of the hall away from the nurses’ station and she is not easily seen by staff. The patient continues to get out of bed without using her call light and eventually falls, resulting in an injury.
Based on the scenario presented, answer the following questions
the risk management tools include
1. Brainstorming
This is a very useful tool for risk management and decision making. The information or ideas are collected from the people who already experience the same problem and solved the problem. this helps to get an idea of risk management and proper decision making.
2. Root cause analysis
In this analysis, identification of the risk which is present in the organization and what is the cause of it. Base on the causes preventive measures are taken. This helps to take proper decisions regarding preventive measures. this helps to develop plan of action to prevent it from happening again.
3. Controlling and directing
proper controlling and directing helps to reduce the risk. this increase in cooperation among the staff and help in proper decision making.

Answer: the risk management issue is that the patient is unaware of the thing present in the environment which is present around her. This is because of failure of the health care worker to give orientation to the surrounding environment to the patient. So as a health worker, on the admission day, the only orientation of the surrounding environment and facilities, rules, and regulations to be oriented to the patient are compulsory necessary.
b)
answer: Orientation to the environment and maintain the safeness of the patient, physical restraint. the nurse should give proper orientation to the patient regarding the use of bell and use of side rail can maximize the injury. It is the responsibility of the nurse to oriented the facilities which are present at the bedside. Advise the patient to ring the bell whenever she needs anything and the nurse also should go for round frequently. Instruct the patient that it is not allowed to get out of bed alone. If not listening then there is a need to do physical restraint.
c)
Answer: In case of needed physical restraint needed for the patient to solve the problem is a big challenge. A health worker cannot directly physically restraint the patient. There is a need to prescribe by the doctor and need to take consent form from the family member. It is because the family member have the right to know why the patient is beitng restraint.
