The World Health Organization defines social determinants as “the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life.” Few healthcare organizations have the resources or political clout to truly eradicate the larger economic, social, and historical constraints that have led to certain groups experiencing long-term disparities and inequities.
To successfully deliver value-based care, improve individual and population health, and reduce healthcare costs, it’s critical for policymakers, payers and providers to address the many social, economic and environmental factors that influence well-being. Many organizations are weaving social determinant data into analytics to more accurately predict the needs of populations and target individuals for outreach and engagement.
