10. The nurse's first priority would be to address Mr. Souza since he is found to be in a secretive state and is carrying something under his shirt. Since he was admitted for a severe episode of generalized anxiety disorder and suicidal ideations, there can be chances that he is carrying any sharp dangerous object under his shirt or could be having some suicidal thoughts inside him. He has also the tendency to harm others. Nothing can be stated clearly so he must be addressed first.
Second priority would be Mr. Canale as he has a bowel movement while sitting in the dayroom and he would not get up and go to the shower. This shows he is going through some very difficult time and is facing serious psychomotor retardation. Leaving him in such condition for a longer period would be very unhygenic for him as well as bad for his deteriorated mental condition.
Third in the list would be Ms. Phillips as she is having bipolar disorder and these patients can be pretty dangerous in their maniac state.
Forth in the list would be Mr. Buchman and the nurse must listen to his thoughts and try to console him and do not let him cry.
Ms. Tobin has been put in fifth place in the list because what she is doing is known to be good for managing depression.
