The HIPAA Privacy Rule establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically.(HHS,2015)

Patients do not have the right to access their own psychotherapy notes or information a covered entity compiled for legal proceedings.

Individuals may be denied access to their protected health information (PHI) without the right to review the denial in the some situations.(Privacy rights clearing house,2018)

Patient donot receive any announcement whenever additions are made to their records
