Total parenteral nutrition

Total parenteral nutrition is the method of administration of essential fluid and nutrients via the central venous catheter.

:Patient with loss of bodyweight less than 10%.

: Patients who can not take food 7 days after post operative days.

:Bowel obstruction and bowel rest

:Poor absorption due to short bowel syndrome.

: Severe hypoalbuminemia associated with dysphagia.

Requirements of fluid include

Water_35 –40 ml/kg/day

Energy:30-45kcal/kg/day

Amino acid :1.0-2.0g/kg/day

Essential vitamins,minerals,trace elements,electrolyte and fatty acid.

Priority nursing intervention for TPN

The goal of nursing intervention is that the maintain nutritional balance,prevent infection and maintain fluid balance.

Most important nursing intervention is that maintain aseptic technique while handling of TPN catheter,solution,and administration of catheter.

Most complication related to TPN is the catheter related blood stream infection.the bacteria is highly grow in dextrose rich environment.as a nurse we have the responsibility to prevent this infection.

Following of aseptic technique while insertion,care and maintenance is help to prevent catheter related blood stream infection and closely monitor for the sign of infection include fever,restless etc,this is the priority intervention for patients with TPN.
