Discuss the underlying pathophysiology of the following patient. Describe the alterations in health and treatment modalities. Please respond thoughtfully and in details of what a nurse would do.

Mrs. Peters is a 33 year old female that is admitted to mother/baby unit after delivering her newborn son. She is 24 hours post-partum. She is a healthy female with no significant medical history and is recovering well. She develops sudden chest pain, panic, and dyspnea. She is transferred to the ICU and is stabilized for a pulmonary embolism. Mrs. Peters is then transferred to your medical floor after she is stabilized. You go in to perform your initial assessment. She presents with the inability to speak or move her left leg.

Medications:

Heparin injection 5000u sq qhs

VS:

BP: 182/98

R:22

T: 97.5

Bloodwork:

WBC: 9.5

RBC: 18.1

Hb: 12.0

Pt: 400

Ptt: 20

INR: 1.5

Pulmonary embolism is most common in prgenancy as well as in postpartum pulmonary embolism occur due to the complication of DVT deep vein thrombosis, or due IV therapy but the most common is DVT..DVT & thrombophlebitis occur in prgenancy due to the venous statis on pressure on the lower extrimites and after delivery it can cause the emboli. .so here im this case the emboli occur due to dvt or during medications.

Heparin:- heparin should be start as early as possible to decrease and to dissolve the clot.

In this the patient having a high blood pressure to decrease the blood pressure nurse should restrict the table salt and sodium containg diet and should also start antihypertensive medication.

But the temperature and WBC count is normal and there is no sign of infection.

Blood work:- WBC in normal level 4500-13500/mm3

RBC in high range (4. 5-5. 5million/mm3)

HB:- in normal range(10-12gm/dl)

Platelet count is also in normal(150-400/mm3)

PTT & INR Is also due to heparin therapy & due to anticogulant therpy.

For communication the nurse should be use the communication skill, alarm bell, and give pen & paper to write always keep around the client.

If the patient cant able to move her leg sand bag should be given and support the leg with pillow and elevate the leg for proper circulation. To prevent dvt occurance.
