The assessment questions to be asked
- When and how or the factors which leads to urine leakage
(patient can experience during cough ,sneezing ,stressful
states)
- Assess for UTI and other medical illness if any because this is
the main cause for stress incontinence (Ask questions about whether
there is pain during voiding ,itching, lower abdomen or back
pain
- How does she manage the symptoms
Intervention
- Teach and encourage to do Kegel exercise as this exercise will
generally strengthen the muscles present in the pelvic cavity
- Encourage patient to do relaxation techniques ,so that stress
can be avoided and reduce the incontinence
- Instruct to avoid caffeine as it is a bladder stimulant and can
increase the stress incontinence
- Educate about the fluid intake and to avoid drinking excess
fluids in pate night as this can increase the risk it f stress
incontinence during sleep
The care given can be evaluated by
- Patient verbalising that she is able to control the bladder and
there is decreased incontinence
- Improved quality of life ,activity, social participation, etc.,
due to reduced incontinence
