Renal system.
Renal system helps in:
- maintaining the blood volume and its components (egs.sodium
,potasium and calcium).
- production of red blood cells
- Maintaining acid base balance (PH) of blood.
- Maintaining the blood pressure .
- Elimination of waste products from blood as urine.
2. List 2-3 reason why a patient would get admitted to
the hospital concerning your system.
Renal calculi-stones formed in the renal system is called renal
calculi,it might need admission in hospital for removal or lysis
.patient will be admitted with symptoms of flank pain ,colic
,hematuria,vomiting etc.
kidney failure-Here the people develops decline in the function
of renal system and exhibit symptoms like decreased
urination(oliguria or anuria),fluid overload (edema),vomiting
,metabolic alkalosis,respiratory symptoms due to overload .
Nephrotic syndrome-nephrotic syndrome is disorder affecting
glomeruli and leads to protienuria.patient exhibits
symptoms like edema ,fatigue etc.
3. Give one (1) Nursing diagnosis. How would you treat
the patient and why
Nursing Diagnosis:Fluid overload related to
Acute kidney failure.
Evidenced with vital
signs:(Tachycardia,Dyspnea,hypertension,desaturation,edema)
HR-100 or above (tachycardia)
RR-20 or above (Tachypnea and Dyspnea)
Blood pressure -150/90 or above (Hypertension)
SPO2-90 or below (desaturation)
Interventions and Rationale
- continuous monitoring of vitals to rule out sudden change in
patient condition needs critical intervention.
- monitor fluid balance by keeping a strict Intake output
chart.it helps to prevent to adjust the fluid intake as per the
output and prevent further overload.
- Physical examination to look for edema,skin integrity,heart
sounds(crackles) etc.
- Provide oxygen to keep a saturation above 94 % (it may include
NIV,or invasive ventilation).As the fluid overload result in
pulmonary edema and desaturation .
- Monitor daily weight to monitor gain in the volume .it will
give an indirect measurement of edema and fluid overload.
- Perform ABG to findout the PH,bicarbonate level.As the renal
failure cause metabolic acidosis and correction will be done based
on the PH values.
- Assist physician in performing dialysis line insertion .A
central line is needed to get a vascular access for dialysis
- Perform blood investigation at regular intervals to rule out
electrolyte imbalance and creatine levels.
- Assist in dialysis prepration.Temporary or Permanent dialysis
may be needed for the patient to maintain the functions of
body.
- Administer medications like
antihypertensives,erythropoietin,calcium carbonate etc as
prescribed.
