Dear student here you have asked many queries, will answer first two only.
1 . Life expectancy is not always the only criteria that describe the healthy society or health of individuals in a society. The society can be healthy when many other pre-requisites are fulfilled that makes a good health reality. The happiness, peace, harmony, better income ,clean and hygienic environment, better sanitation facilities, good availability of sources of entertainment and other activities ,awareness about physical fitness and much more. Even if the country has better life expectancy but is always involved and suffering from chaos and much more the results will always be negative to its balance. Though it is reality that better life expectancy countries like Japan are following every area that makes life expectancy a reality like better food culture, clean environment, less interference into natures boundaries, better health care facilities, peace etc.
2 Fear based campaigns work to decrease unhealthy behaviors is sometimes a reality like the scary pictorial warnings on cigarette packs or tobacco packs that depicts the health hazards of using the drugs or tobacco. The substance users or addicts may stay away if they understood the health issues behind using any health hazardous item.
