Clinical document improvement is a process of improving the helath care records of the patient for their better outcome. For establishing an effective clinical documentation improvement program it is necessary to make a good work flow by  the clinical documentaion specialist.(cds)

▪Evaluate and analyse the patient records

▪Go through the records for proper and improved documentation.

▪Making report of new cases from each department

▪Daily discharged cases are reviewed and documented it properly for future use.

▪Cds perform the chart reviews by the  end of their duty and he/she will make sure that all the details in the document are accurate. In case of any missed details they will contact with the particular department.

▪Follow up of each document and unanswered queries.

▪Contact the patients insurance company if needed.

▪Ensure all documents are compliance with the law

▪Train helath care workers about standard procedure for documentation.
