Mary, a 25-year-old G1P1 single mom who is 2 days postpartum. She has minimal support and is returning to work in a week if her doctor releases her. She is currently bottle feeding and ready for discharge. She is worried how she is going to make everything work. What are your concerns as her nurse and what would you include in her plan of care? How would you prioritize her nursing care?
Mrs. Maya , a 25year old post partum mom has minimal support and she plan to returning to work after a week. Having a baby is a lifechanging event and post partum care is important and crucial as a new parent.
As a nurse the first priority and conern is about baby care. Exclusive breast feeding is important for newborn. As a nurse i can advice Mrs. Maya to go for exclusive breast feeding because currently she is on bottle feed. I can advice her to breast feed the baby and when she starts going for work she can restart bottle feed.
Initial attempt at nursing can be painful, tenderness and discomfort. Patient may experienced by sore or cracked nipples, it can be reduced by expressing a few drops of breast milk on the nipple after each nursing to prevent crackles and further irritation.
Advice her to wear a clean Bra and keep nipples always clean.
Baby bath and umilical cord care is important
Apart from baby care other plan of care include Mrs. Mayas body care include bathing, sitz bath to prevent infection related to post partum. Costipation and hemorrhoids are more common due to pressure of uterus during pregnancy and delivery. Normal bowel habits should be maintained by adding extra fluids and fibre rich diet. For hemorrhoids sitz bath can be provided.
Episiotomy is an incision occur during child birth , episiotomy care by regular dressing and sitz bath can be provided.
Post partum mild exercise after 2 weeks to regain muscle strength. More straneous exercise should be delayed for 6 weeks.
A healthy balanced diet with wild variety of fruits and vegetables.
sleep and rest is important 8 hours of sleep and proper rest is important
Sexual intercourse can be avoid for few weeks and if necessary contraception can be used as per doctor advice.
Post partum depression is common in many mothers due to stress and anxiety. adjusting a new life with a child for the first time, feeling of isolation, life stress. financial instability , physical body changes are some of the common reason for post partum depression. providing psychological support , parental education , educating about body changes are common and can be rgain the body by exercise and diet and family support hepls the patient to regain confidents.
Follow up visit to hospital is important
