Nesojejunal tube is placed after numbing gel on the nose. A small tube into the jejunum (small Bowel) using fluoroscopic (X-ray guidance).
The nurses encounter tube blockage by many times, it is a very common issue with patients receiving jejunal feeding.
It is a flexible plastic tube inserted through the nostrils, down the nasopharynx, and hen into the stomach or a few upper portions of the small intestine.
Nasojejunal tubes are used to:
1. Remove gastric contents or to remove the stomach toxins.
2. Deliver nutrients to the patient via a feeding pump.
a. Rotating the tube and then inserting it
Explanation: If a nurse fell slight resistance, as a nurse advance along the nasal passage, rotate or twist the tube slightly, apply downward pressure, and continue trying to advance the tube. If the problem continues or resistance is felt, remove the tube and allow the patient to rest before trying again in the other nostrils.
b.Administrating anesthetic to the patient.
Explanation: The patient should sit up without chin up. An appropriately sized tube-taken and the tip of the tube is lubricated by smearing anesthetic gel or aqua gel. It is a drug used under the prescribed conditions and precautions taken such as checking for allergy.
c.Inserting the tube through the other naris.
Explanation: If we suspect that the tube placement is inhibiting the flow, stop the procedure, and reevaluate tube placement from another naris.
d.Placing the patient in the side-lying position.
Explanation: This position facilitates the downward flow of medication into his stomach and prevents the esophagus reflex. the patient never allowed to lie flat for a long time because it increases the risk of aspirating stomach contents.
e.Asking the patient to swallow while the tube is inserted.
Explanation: Provide the patient with pure drinking water and a straw for sipping water. If the patient is not fluid restricted. Sipping water with the help of straw to initiate the swallowing reflex and facilitate the passing of the Nesojejunal tube.
Thanks and God bless You.
