Mr. Chung has smoked two packs of cifarettes per day for 50 years. ABGs reveal a liw PO2 and a high PCO2 consitent with chronic lung disease. He is diagnosed with an acute exacerbation of chronic bronchitis. He is placed on oral antibiotics and long acting bronchodilators by metered dose inhaler. What teaching could you offer to help Mr. Chung mobilize and expectorate secretions?
Chronic bronchitis is the inflammation of the bronchioles due to chronic irritation which cause increased mucus production and obstruction. The symptoms are cough with sputum production. Smoking is one of the major course of this condition. Antibiotics and bronchodilator are used to treat the condition. However cessation of smoking is very important to control the recurrent exacerbation of the condition.
The patient teaching should mainly focused on
1) Teaching deep breathing and coughing exercises.
2) To drink warm fluids and increase the oral fluid intake.
3) Do not use alcohol or Caffeine containing drinks.
4) Stop smoking.
5) Advise to avoid airway irritants such as dust, fumes, air pollution exposure, and vapors.
6) Use of face mask during outdoor activities.
7) Early prevention of pneumonia and flu buy taking timely vaccines.
8) Importance of regular and complete use of medications.
9) Proper use of metered dose inhalers.
10) How to identify the symptoms of exacerbation of condition early and consult the doctor.
