A. Since , it would be and also it's not the standard to ask some one who is not able to move and without assistance directly that why are not maintaining hygiene in terms of bath , hair care ,oral care etc .

I will firat develop rapport with her and then ask slowly about her daily activities , how she maintain to do all work alone etc . In this i would be able to know about her day care habits .

B. To address her deficient hygiene ,i would make her realize that she is not looking clean and would help her to in taking bath , mouth care , perineal care ,hair care etc and will explain about the importance of keeping herself clean .

C.The questions regarding her skin condition is appropriate because her skin is looking dry and to confirm whether she having any other skin diseases .

d. She indicates that her favorite soap is deodorant soap. Why might that not be the best choice for her?

A deodorant soap may or may not contain an ingredient that kills or inhibits the growth of odor-causing bacteria and causes build of bacteria resulting in bad odour , also causes overdryness of the skin
