The process of obtaining informed consent is complex, challenging, and warrants focused teaching, observation and feedback.

Suppose a 25 years old female patient Mrs.M arrived hospital with abdominal pain. After performing diagnostic procedures doctor said she is having appendicitis and appendectomy have to be done.

Now, a nurse goes to patient side for informed consent.

Nurse- hello, Mrs M. How are you today?

Patient -hello, I am feeling quite better today.

Nurse- Oh! That's good. So, we have performed various diagnostic test for you. On the basis of the reports it is conformed that you are having problem in your appendix. You have acute appendicitis. The appendix is a pouch-like structure attached at the start of the large intestine that has no known purpose.In this condition appendix becomes inflamed and filled with pus, causing pain.

Patient- Oh! My god!

Nurse- No need to worry abour. It is a common problem.To treat is ,we have to do appendectomy. It is also a very common operation . Through this operation we will remove your appendix. It will be a laparoscopic. That means it's done without a large incision. Instead, from 1 to 3 tiny cuts are made. A long, thin tube called a laparoscope is put into one of the incisions. The appendix is removed through one of the incisions.On average, the surgery costs about $33,000

Patient- Ok.

Nurse- As we are going to perform a operation some complications may arise like hemorrhage, infection,blocked bowels, injury to nearby organs can occure. But medicos will try their best to avoid complication.

Patien- okk.

Nurse- This surgery will take about 1 hour.

Patient -ok.

Nurse- You will probably be able to get back to most of your normal activities in one to two weeks time. These activities include showering, driving, walking up stairs, working and engaging in sexual intercourse.

Patient - ok.

Nurse- So, Mrs. M. Do you have any questions?

Patien- No.

Nurse- Did you understand everything I said?

Patient- yes.

Nurese- So ,Will you do the operation? Are you giving consent to perform the operation?  

Patient - Yes, I am ready. But I am just little bid worried.

Nurse- We are with you always. No need to worry. I will informe you the date of your surgery.

Patient - ok, thank you.

Nurse- Meals cannot be eaten 24 hours before operation to prevent any chance of aspiration. We also have to do shaving of the operational side.

Patient- ok.

Nurse- ok, then. Will you please sign here? This is a concent form.

Patient- Sure.

Nurse- ok, then. Take rest.
