2. A 68-year-old female has an LDL level of 150 mg/dL with a history of diabetes and hypothyroidism.
a. This client is started on a statin drug. Discuss your potential nursing assessments and implications for nursing care.
b. Discuss your teaching priorities for this client.
Ans 1a)-LDL level should be less than 100 mg/dl.
100-129mg/dl are acceptable for people with no haelth issues but may be of more concern for those with heart disease
130- 159 mg/dl is borderline high
160-189 mg/dl is high
Hence result shows she is on borderline high level which needs to be control
b)-Cholesetrol- is a type of fat found in your blood and in all your body's cells.It helps protect nerves,makes cell tissues, and produces certain harmones.But if too much cholestrol builts up in your blood vessels,it can block blood flow and cause a heart attack or brain attack.
There are two types- LDL- bad cholesterol -low density lipoprotein
HDL- good cholesterol- High density lipoprotein
High amount of LDl can lead to different health issues,can cause heart attack.
c)Metabolic Syndrome - A cluster of conditions that increases the risk of heart disease,stroke and diabetes.
Metabolic syndrome includes high BP,high blood sugar,excess body fat around the waist and abnormal cholesterol levels.The syndrome increases a person's risk of heart attack and stroke.
Ans 2-LDl -150mg/dl -borderline high
a)Statins- these are your cholestrol lowering drugs like atorvastatin.Statin use caused significant decrease in TSH indicating a need for a lower thyroid medication dose.
Having an underactive thyroid can lead to an increased cholesterol level and treating hypothyrodism may cause your cholesterol level to decrease without the need for statin.
There are high chances thyroid level will decrease more.
b) Some substance can interact with statins- with some statins- drinking grapefruit juice or eating grapefruit is a bad idea.Grapefruit juice can cause that statin to stay in your body much longer and the drug can build up.This can increase the risk of muscle breakdown,liver damage and even kidney failure.
-Make her aware that she can experienced muscle weakness.
-There are increased risk of developing diabetes.As she is a diabetic patient so she should have to take good care of this.
