Piya Jordan is a 68 year old patient who was admitted to the hospital to have surgery to remove an abdominal mass. She underwent a colectomy yesterday removing a mass from her right ascending colon. She has a large abdominal incision with a clean, dry and intact dressing, but fortunately did not require a colostomy. She is on the post operation surgical unit and is requiring post-operative nursing care. She is alert and fully oriented to her surroundings. She has a nasogastric tube in place which has drained 10cc of yellow drainage in the past 8 hours. She is complaining of abdominal discomfort and she states “I’m sick to my stomach and my belly is full”. Plan of action

• Check vitals and BP of patient and make sure she is not having Hypotension
• As she is feeling abdominal fullness she might be having gas or bloating so ask to lie down flat that help to release gas
• Analgesics can be given to manage pain
• Tight dressing can be avoided
• Flushing if Ng is indicated if feed is given
• Give basic hygiene care
• Provide food that avoid causing bloating
• Sterile dressing can be done

Question no 2)

• Slight delay in GI motility as a result of post operative ileus . This is a major complications
• Hemorrhage can occur as a complications
• Nause and vomiting
• Dehiscence of surgical wound
• Infections
• Fever
• Deep vein thrombosis and atelectasis due to post operative rest

Evaluation

Patient is expected to stay free from complications like pain, hemorrhage and infections.

Patient gets relived from risk for infection

Patient does not develop post operative complications like atelectasis and DVT
