While conducting an eye test, the healthcare professional should observe the following :
1. History of any fall: Poor eyesight can be linked with an increased incidence of falls and fractures particularly hip fractures in the geriatric population.
2. Check for any discharge, redness, or bruising in and around the eyes: This could be a sign of conjunctivitis, cyst, or problems with eyelid muscles.
In addition to this, the nurse must also assess the eyes for any abnormalities such as swelling of eyelids, the color of the sclera, and conjunctiva and pupil size. Check for Strabismus and anisocoria.
The cranial nerves ( III, IV, and VI) responsible for eye function can be checked by looking for nystagmus, reactivity to light, and accommodation.
