Isomer are the molecules that have the same chemical formula but a different arrangement of atoms.

Hexokinase phosphorylates glucose to glucose 6-phosphate. Since both are different molecules. So they are not isomers. Both have a different chemical formula. So option B is wrong.

Transketolase transfer ketone group from one molecule to other molecules. So option C is also wrong.

Lactonase carried out the hydrolysis of  -6-phosphoglucono-δ-lactone to 6-phosphogluconolactonase. Both have a different chemical formula. So option A is also wrong.

Phosphopentose Epimerase convert ribulose 5-phosphate to xylulose 5-phosphate. Both have the same chemical formula and have a different arrangement of atoms. So correct answer is option D- Phosphopentose Epimerase.
