Since the patient needs 5000 units of heparin for subcutaneous injection, 1ml is the amount required.
Amount that the nurse will administer=1 ml
*Doctor's order is : amount of replacement fluid for urine output of 100ml is 25ml of water through nasogastric tube every 4 hrs.
Here,given that patient's urinary output for the last 4 hours is 500ml.so,the amount of replacement fluid needed corresponding to this output is 5×25ml of water.That is 125 ml of water.(for 100ml of urine --->25ml of water,then for 500ml of urine output(5×100ml)--->5×25ml=125ml of water )
So,amount of replacement fluid=125ml of water
*Doctor's order is morphine sulfate 2mg IV .
Morphine sulfate is available as 4mg/ml.That means each ml contains 4mg of morphine sulfate.since the patient needed half of the amount(2mg means half of 4mg),the volume needed is 0.5 ml(0.5×1ml=0.5ml)
The amount of medication needed=0.5ml
*4)Doctor's prescription is ke!ex 500mg PO BID.That is 500mg per orally 2 times a day.
So patient needs 1000mg of ke!ex per day(2×500mg).
It is available as 125mg/ml.it means 1ml volume contains 125mg.
So,volume which contain 1000mg will be=(1000×1)÷125=8ml per day.
The nurse should administer 8ml per day or 4ml per dose(two times a day constitutes 8ml)
