The shock is applied to set up a current across the heart, this delivers a small amount of energy to the heart which causes it to work again.

The defibrillators work by applying a moderately high voltage across the heart, this causes a current to get set up across the heart which then kicks starts the heart into working again. This is usually used in the case when the person is undergoing cardiac arrest ie. when the heart suddenly stops due to electrical issues in the heart.

Normal devices deliver monophasic shock where the current will be traveling only in one direction, whereas in Biphasic shock the device is able to deliver current in both the directions. There is a huge advantage in biphasic waveforms as compared to monophasic waveforms, normal biphasic waveforms with lower energy as compared to monophasic forms are enough to improve the heart condition. This waveform is safer than the monophasic waveforms.
