During times of stress or illness, patients may require a sliding scale insulin regimen. what is a sliding scale regimen? Why would someone under stress or experiencing illness require more insulin?
A.
Sliding scale regimen in simple form is daily body requirements of insulins via body to Maintain normal blood glucose level. In this regimen premeal and bedtime time high blood glucose level is corrected.
In sliding scale the progressive increase of premeal or bedtime blood glucose due to insulins resistance or lack of insulins production. it is required during the time of illness or stress.
B. At the time of stresse or illness the stress hormone(cortisol from adrenal gland) production increases. This interefer the production of insulins or insulins resistance to the effector cell. During the illness the blood glucose level goes up and lack of insulin leads to sometime ketone bodies formation. To prevent any hyperglycaemic shock and ketone body formation the patient has been administered insulins from outside to Maintain normal blood glucose level during stress and illness
