1. There are several safety risk in home. Some factors that can contribute to client's injury risk are environmental factors such as lacking enough space , slippery floor ,uneven or unstable floor and extreme cold or hor environment. Factors such as bearing heavy load , inappropriate posture may be due to bed or chair, absence of handrails and other assertive hardwares , also electrical appliance improper functioning or damaged socket can cause burn injury.

2. Actions nurse should use to prevent IV infections are proper handwashing before preparing drug , administration of drug . provide antisepsis with chlorohexidine. Disinfect ports and hubs by wiping it with antiseptic solutions such as chlprohexidine. Use aseptic techniques and proper cleanliness before inserting IV line. Choosea n appropriate insertion site without any presence of lesions or wound to avoid hematogenous spread of infection. Open syringes and needle with proper sterile methods also load medication with maintaining sterility and to avoid risk of unsterility which may cause IV infections. Always wear clean gloves before administration of IV solution . Monitor for sign of redness and swelling at site of IV therapy .

3. Three seizures precautions nurse should implement are

a) Set up a suction ,have an oral airway , oxygen and suction readily available.

b) keep bed in lower position with siderails up to prevent injury also monitor continuously to seizure patient thus need to be provided bed near nurse station. If possible stay with patient to protect from injury and observe for seizures activities.

c) Don't restrain the patient and make sure nothing is in mouth ,pad the siderails if possible . Also keep AMBU bag in room .
