The home health care nurse is visiting Mr Simpson, a 62 year old man with a significant history of anigina pectoris. During the visit the nurse assesses Mr Simpsoms current status, including his vital signs, activity level, and dietary intake. Mr Simpsons medications include sublingual nitroglycerin as needed for chest pain, metropolol, Cardizem, and ticlop;idine.
!. what are the rationales for the prescribed medications?
2.Mr Simpson continues to smlike despite his disease process. How does moking increase Mr Simpsons chances of anigna episodes?
3. The nurse reviews the correct procedure for taking nitroglycerin for chest pain, includes what information?
4. The nurse uses the PQRST acroynm to assess for symptoms of angina. What is the nurse assessing?
1. Prescribed medicationa are nitroglycerin sublingual, metaprolol , cardizem, ticlop, idine. Rationale for use of these drugs are *Nitroglycerin helps to open up the blood vessels and allow blood to flow . * Metaprolol used to reduce the high blood pressure as it was beta blocker, chest pain due to high blood pressure is reduced. * Cardizem is calcium channel blocker it works by relaxing blood vessels in the heart and lower the heart rate and reduce chest pain. *Ticlop is anti coagulate formation of harmful clot and allow for free blood flow and reduce chest pain.
2 .Moking is a term for mix of marijuana and tobacco ,typically smoked with a bong ,or water pipe. It increase the. Chance of angina by formation of plaques in blood vessels.
3 . There are directiona to use nitroglycerin for the angina. *One tablet placed under the tongue or between the cheek and gum at the first sign of an angina attack. *one tablet may be used to every5 minutes as needed ,for up to 15 minutes . *Do not take more than 3 tablets in 15 minutes . *To prevent angina from exercise or stress ,use one tablet 5 to 10 minutes before the activity.
4.PQRST is acronymused in nursing assessment of pain. P - stands for precipitating factors. Q -quality of pain. R- radiation of pain. S- Subjective description of pain. T - Temporal nature of pain. By using these things nurse has to assess the pain .if it comes to angina we need to assess
*need to check for precipitating factors for the pain those are stress, use of alcohol and smoking which cause the cardiovascular problems. And need to check what aggrevates that pain like bending, lying down, walking or standing and what relieve that pain like medications , changing positions. If it agrevates due to heavy working, stre its related to angina , if it realives by medication like aspirin or nitroglycerin it was related to angina. * Quality of pain in words like sharp, dull , stabbing, burning, nauseating , twisting or stretching type pain .Angina pain will be sharp, dull pain, if it was burning pain it related to gastritis. * Region or radiation. ,if pain was related to angina pain will be radiating to left shoulder and left hand . if pain only radiating to midline of back its related to gastritis. *Severity of pain should be measured by assessing pain level by pain scale. *Timing of pain like weakly or monthly once or after doing heavy working we need to assess.
