Susan Jackson is a 66 years old African American women who is admitted to acute rehab for continuing care of a right below the knee amputation. Mr Jackson had a surgery 2 weeks ago the wound is not healing properly. Mr Jackson has a history of type 2 diabetes mellitus, hypertension, hyperlidemia and obesity.
What are you primary concerns for this patient and what assessment and interventions would be associated with your concerns and why ?
My primary concern for this patient is to avoid surgical site infection.
She is prone to get surgical site infection as she is a known case of Type 2 diabetes. Type 2 diabetes mellitus slows down our body's ability to fight infections . This is because high blood sugar levels helps the bacteria to grow effectively and causes infection.
Also, the other concern is she needs assistance in activities of daily living.
Assessment for my concerns are:
* Patient is a known case of diabetes mellitus with other comorbidities which puts the patient in risk of infections.
* Also, for the surgery done 2 weeks back, the wound is not healed properly which can be a sign of infection.
* Assess for any signs of infections.
* She is obese and had knee amputation, which makes her to get assistance.
Interventions include:
* Providing proper dressing for the surgical site.
* Administering antibiotics as per order to prevent infections.
* Provide a clean environment.
* Monitoring vital signs
* Follow proper handwashing techniques while taking care of the patient.
* Provide her assistance in activities of daily living.
* Administer anti-diabetic drugs and maintain her blood sugar at normal levels.
