The nurse is admitting a 68-year-old patient with a history of ovarian cancer to the medical unit. She had surgery 2 months ago and has had pain ever since the surgery. She reports that she has been taking oxycodone at home, but that the pain is “never gone”

Pain is a physical discomfort ranging from mild to severe ,usually caused by injury, illness or a nerve condition.
The visual analogue scale is a psychometric response scale range 0-10 ,it is a measurment instrument for subjective characterstics that cannot be directly measured .
When responding to visual analog scale respondent specify their level of agreement to a statement by indicating a position along a continous line between two end points.
The nurse should document that patient have severe pain acc to visual analog scale
