I'm doing an assessment assignment of the lower
extremities, I need 2 sentences for each question listed below.
There should be a total of 10 sentences.
1.Inspect LE for joint swelling/deformity
2.Inspect LE for range of motion
3. Assess for pain/tenderness
4.Checks LE push/pull bilaterally
5. Checks ambulation /Gait
Logical sequence of exam (IPPA) and organization of assessment.
Greetings of the day!
Answer:
(As this is a practical question, we can solve this question with a help of any clinical example.)
for example, for the assesment of lower extrimities for a 82 year old female patient with congestive heart failure:
1.Inspect LE for joint swelling/deformity
-inspection- the overlying skin of the lower extremities, is shiny and taut, puffiness is observed. there are no wrinkles and no skin folds or creases.
-palpation- The palpation component of the assessment confirms the presence of swelling over the anterolateral aspect of the ankle and in feet. it is a 1+ (2-4mm) bilateral moderate pitting edema.
Otherwise, no abnormal findings were present.
2.Inspect LE for range of motion
-Active- No abnormality detected in the bilateral leg extensions, marching, toe raises, heel raises, foot press, knees flexion and knees extension.
-Passive- No abnormality detected in the bilateral ankle & toe dorsiflexion and extension, hip flexion, hip external rotation, hip abduction, and straight leg raise.
3. Assess for pain/tenderness
-palpation- bilateral achilles tendons, knees and hip joints are painful on palpation due to age factor.
-percussion- bilateral achilles tendons, knees and hip joints are painful on percussion due to age factor.
no abnormal findings were present.
4.Checks LE push/pull bilaterally
-push- With the help of an dynamometer (flexor) ,bilaterally, the strength test was performed to the lower limbs, and asked to push and the patient complains of weakness and tremors.
-pull- With the help of a dynamometer (extensor), bilaterally, the strength test was performed to the lower limbs, and asked to pull, and the patient complains of weakness and tremors.
Otherwise, no abnormal findings were present.
5. Checks ambulation /Gait
- Ambulation- The patient is able to ambulate with the help of ambulatory aid (a cane).
-Gait- The patient has a antalgic (painful) limb gait is present due to joint pains.
Otherwise, no abnormal findings were present.
