It’s important to submit accurate and current codes so that quickly and efficiently process claims can be generated for accurate and appropriate reimbursement. Medical billing and coding are two of the most critical components in this. Accuracy in both of these areas is critical to patient safety, quick payment and efficient operations. It requires skilled professionals who can read, interpret, record and track complex medical information quickly, maintain patient confidentiality and strong attention to detail. Medical coding starts with a patient visiting the physician’s office or hospital. When the patient visits, detail of the patient’s medical record and why they furnished specific services, items, or procedures are made. Medical billing begins when a patient registers at the office or hospital and schedules an appointment. it requires complete forms and they confirm patient information, including home address and current insurance coverage. Providers use clinical documentation to justify reimbursements to payers when a conflict with a claim arises. If it is not properly documented in the medical record by the staff, the organization could face a claim denial & could also be subject to a healthcare fraud or liability investigation if they attempt to bill payers and patients for services incorrectly documented in the medical record or missing from the patient’s data altogether.
