  1. Calculate a client’s intake and output (I&O) in milliliters (mL) from 0700 to 1500.

Breakfast:        3/4 cup of coffee

                        3 oz glass orange juice

Lunch:             4 oz diet soda

                        6 oz chicken broth

Voided:           200mL at 1000

200mL at 1400

            Emesis:            125mL at 1300

IV fluids:         Lactated Ringers @ 100 mL/hr

Total 8 hour intake =   mL

Total 8 hours output = ml

  1. The nurse receives an order to give atropine 300mcg SQ now. The medication is available in 1mg/mL vial. How many mL should the nurse administer? (Do not round)

300/100 = 0.3 ml/hr

  1. Calculate the IV infusion rate using a pump (mL/hr) for 1L LR over 8 hours.

1000/

  1. Calculate the blood transfusion rate using a pump (mL/hr). 1 unit PRBC (225mL) over 2 hours. (round to the nearest whole number)

  1. Calculate the following IVPB infusion using a pump (mL/hr). Zofran 20mg IVPB every 6 hours infused over 30 minutes. The IV bag contains 20mg in 100mLs NS.
  1. How long will it take to finish infusing 1L of D5NS 100mL/hr?

  1. The nurse receives an order to give heparin 1,500 units/hr. The IV bag contains 20,000 units heparin in 500mL D5W. Calculate the IV rate in mL/hr. (do not round)

Solutions

Expert Solution

1) Calculate clients intake ,output from 0700 to 1500

3/4 cup of coffee= 177 ml (US CUP)

1 ounce = 30 ml

3 OZ glass orange juice = 30*3=90 ml

4OZ diet soda =30*4=120 ml

6 OZ chicken broth=6*30=180 ml

iv fluid at 100ml/hr for 8 hrs=100*8=800 ml

total intake =177+90+120+180+800=1367

Total 8 hr intake =1367 ml

amount voided =200+200=400 ml

emesis=125 ml

total 8 hr output =400+125=525 ml

2) 1ml=1000 mcg

dose to be given=300mcg , dose at hand =1000mcg

dsired dose=ordered dose / amount at hand * quantity

300/1000*1 =0.3ml

The nurse should administer 0.3 ml of atropine SQ.

3) iv infusion rate using a pump 1litre RL over 8hrs

1 ltre =1000 ml

iv infusion rate in ml/hr = amount to be administred in ml / time in hr

                                 =1000/8 =125 ml/hr

The answer is 125 ml/hr

4) Blood transfusion rate using a pump in ml/hr 1 unit PRBC over 2 hrs

1 unit PRBC =225 ml

time =2 hrs

the transfusion rate in ml/hr =amount infused in ml/ time in hr=225/2= 112.5 ml/hr

the answer is 112.5 ml/hr

5) Zofran 20mg in 100ml is available

time =30 minutes =1/2 or 0.5 hr

infusion rate in ml/hr= 100/0.5=200 ml/hr

the answer is 200 ml/hr

6) how long it will take to finish 1 litre of D5NS 100ml/hr

amount to be infused=1000 ml

infusion rate in ml/hr= 100ml/hr

time to finish =?

time= amount to be infused in ml(volume)/rate in ml/hr

      =1000/100=10 hrs

the answer is 10 hrs

7) 20,000 unit heparin in 500 ml is available

20000/500= 40

so 1ml soution contains 40 unit heparin

amount to be infused 1500unit/hr

so 1500 unit contains how much ml?

1500/40= 37.5 ml

so the iv rate in ml/hr to infuse 1500 unit/hr is 37.5 ml/hr
