National safety and quality health services standard are standard set by the governance to make safety high quality care in the health care delivery system.
Screening and assessing risks, preventing falls, and communicating with patients & cares are coming under standard 10 ( prevention of fall and harm from falls.
Criteria to achieve the standard 10 includes
1. Screening and assessment risks
Screening and assessment risks of fall should be done start from the patient on presentation, during the admission, and clinically advice to do the screening and assessment.
The criteria can be achieved by
a. used of best evidence best practice tools for the screening the patient.
b. conduct a comprehensive risk assessment for the patient who are identified as risk for falls in the initial screening process.
2. Preventing falls
prevention strategies are used for the patient who are at risks for falls.
The criteria can be achieved by
- development and planning of multifactorial prevention of falls to address the risks of fall identified in the assessment
- Referred the patient who are at risk of falls to appropriate services during the discharge process.
3. communicating with the patient and the carer
Explain the identify risks of fall to the patient and the carer then encourage for the active involvement of the patient and carer during the falls prevention plan.
The criteria can be achieved by
- Informed all the information regarding the risks of fall and preventive strategies for the patient to the patient and the carer.
- develop the fall prevention plan in co operation or collaboration with the patient and the carer.
