Any ideas on nursing diagnosis? a couple of ideas would be appreciated
Sherry, 33, HIV/AIDS. Recently discharged after treatment for pyelonephritis, Discharged home on Hospice Care Services. No longer employed since diagnosed. Lives with extended family on the 6th floor of a city housing apartment. Has no insurance.
HIV/AIDS is a chronic disease condition caused by human immunodefiency disease.As per the virus name victim' s immunity level will be decresed day by day and finally death occurs. Needs symptomatic treatment and anti retro viral therapy to decrese the viral load in the body. Can be affected in all body systems and functions. Patient may experince lethargy and weaknss.
This type of cases patient is at a high risk of infection.Pyelonephritis is a bacterial infection common in HIV patients. So the initial nursing diagnosis is (a) RISK FOR INFECTION.
b.. INADEQUATE NUTRITION DUE TO INSUFFICIENT INTAKE OF FOOD .
c. ALTERATION IN BOWEL REGIMEN DUE TO DIARRHEA CAUSED BY PATHOGENIC ORGANISMS IN THE GI TRACT.
d. ALTERED OR IMPAIRED SKIN INTEGRITY DUE TO BOWEL IRRITATIONS AND SKIN RELATED INFECTIONS.
e. ALTERATION IN MENTAL STATUS DUE TO FINANCIAL INSECURITY AND THE DISEASE CONDITION (NO JOB AND FAMILY EXPENDITURE).AND ALSO NO INSURANCE COVERAGE FOR THE TREATMENT.
f. ALTERATION IN ACTIVE AIRWAY FUNCTIONS DUE TO GENERALISED WEAKNESS AND LETHARGY. PULMONARY SECRETIONS CAN BE INCRESED DUE TO WEAK COUGHING AND LACK OF ACTIVE MOVEMENT.
g .COMPLIANT OF PAIN DUE TO PYELONEPHRITIS.
i. ALTERATION IN MOBILITY DUE TO WEAKNESS .
j. IMPAIRED SOCIAL SECURITY DUE TO THE DISEASE AND THE FINANCIAL INSECURITY.
