To developing a first five year new health service plan offers two primary health service including mental health service and physical health services. Mental health service provides assessment and intervention for people with mental illness and primary health service provides general health management.This includes assessment, treatment,emergency and referral service for old people.

  • Aged care assessment team
  • Community assistance and support program
  • Falls and falls injury prevention program
  • Geriatric department
  • Out patient geriatric services
  • Rapid assessment of the Detiriorating Aged at Risk (RADAR)
  • Social work- Aged care inpatient services
  • Transitional therapy and care program

I can give telehealth service with physicians and provide medical insurance.

2. The criticality in terms of assistance and support towards the 'older' aged ,that is those aged over 75-80 years of age.Seperate support team is needed to care and assess this group of people.Rising the patient volume an d labour shortage is the another problem.But the new digital healthcare technologies can help bring down cost,improve care qulaity of old age people.Telehealth assisted living is also most useful for aged people

3. Future long term care consists of the services and support given to the patients with complex needs due to illness,disability or a mental condition and is typically provided thos eaged 65 and above.

4.Arrange a assessment team to improve the functional capacity and assessment of the plan.This team consists of physiotherapists,occupational therapists,allied health assistants,social workers,dietiticians,care support staff and case managers.This team can study the various aspects of this plan, whether it is effecttive or not, what are the channges need to make and submit a report.In this report, we can verify the effectiveness of the paln.
