The common drugs used to treat peptic ulcer disease are
1. Antibacterial agents
The aim is to eleminate Helicobacter pylori infection. The drug regimen used consists of clarithromycin 250 mg twice a day plus metronidazole 250 mg 4 times daily for 10 to 14 days.
A triple therapy consists of 2 antiinfective agents and a proton pump inhibitor.
A quadruple therapy consists of proton pump inhibitors, Tetracycline, Metronidazole and Bismuth Salt
2. Hypersecretory agents
The aim is to reduce gastric acid secretion. These include
H2 receptor antagonist eg. Ranitidine 150 mg twice a day,
Prostaglandin analogs eg. Misoprostol. It reduces gastric acid secretion and stimulates production of cytoprotective mucus
Proton pump inhibitors eg. Omeprazole 40mg once a day before breakfast or dinner. Inhibition of H+, K+ and ATP ase
Anticholinergic eg. Dicyclomine hydrochloride. Inhibition of gastric acid in large doses.
Antacids eg. Gelusil, Riopan . Twice a day, before food. Increases gastric ph to reduce pepsin activity and strengthen mucosal barrier.
3. Mucosal barrier fortifiers
Eg.Sucralfate. It forms a viscous gel that adheres to ulcer surface. Give on empty stomach.
4. Corticosteroid. Eg. Hydrocortisone. To stimulate parietal cell regeneration.
5. Vitamin B12 to treat pernicious anemia
6. Bismuth containing preparation. Eg. Colloidal Bismuth and the mechanism is not known.
