Since 1965, Community Health Centers have delivered comprehensive health and social support services to more than 22 million Americans. It provides comprehensive high quality primary health care as well as helping individuals get access to health care. It exists to serve those who have limited access to health care although all are welcome. CHC provide services available to all with a sliding fee scale based on ability to pay (family size and income) for those who live at 200% or below of the federal poverty level. Community Health Centers are funded through a combination of federal and local grants and payments from patients and insurance companies.

The growing awareness will play a significant role in eliminating the people's hurdles in obtaining preventive health care services. The self-efficacy level in individuals can be promoted through social cognitive theory so that they can be become more aware towards their health care needs as well as more confident in suggesting others the appropriate level of health care consultation needs. We can also motivate individuals to help our campaign for social welfare in the health care sector. Also , we shall be focused to achieve ability to deliver the patient centered health care services to the mass with the help of this theory.
