Discuss Human factor analysis method of reporting medication error in clinical setting.
Medication error is a severe patient safety event in the United States. Medication error reports collected by patient safety organization's provide an opportunity to analyze and learn from previous errors.
A couple of studies have reported that inpatient hospital medication error rates are between 4.8% to 5.3%.The medication error is the main cause of the adverse drug events, which could cause substantial adverse consequences, including but not limited to the patient harm, unnecessary hospital admission.
Considering high frequency of medication error with resultant patient harms their prevention is a worldwide priority for health care systems.
However the current workflow analyze the error reports is labor intensive and time consuming. To reduce the workloads for clinicians and save time. The pipeline was proven functional in two tasks i. e. identifying the error and error types and error cause.
