Given the following clinical scenarios, select the proper aerosol generator and aerosol delivery device as well as the reason why you choose it:
A 4-month-old with respiratory syncytial virus
An 18-year-old asthmatic who is sensitive to the propellant in the MDI
AEROSOL GENERATOR -a device for producing airbone suspensions of small particles for inhalation therapy
AEROSOL DELIVERY DEVICE - a device used for assisting in the delivery of medications by inhalation
It is recommended that selection of the appropriate aerosol generator and interface be made based on patient's age, physical and coginitive ability, cost and availability of prescribed drug for use with a specific device
The primary mechanism of aerosol deposition within the respiratory system involves three distinct factors of deposition ; inertial impaction ,gravitational sedimentation,[settling]and diffusion
nebulizers and pressurized metered dose inhalars [pMDIS] with valved holding chambers are suggested for use with children less than or equal to 4 years old and adults who cannot correctly use amouth piece ,aerosol masks are suggested as the interface of choice
TYPE OF AEROSOL DELIVERY DEVICE
small volume nebulizer
pressurized metered dose inhaler
dry powder inhaler
IN THE CASE OF 4 MONTH OLD WITH SYNYTIAL VIRUS
METERD DOSE INHALERS WITH ALBUTEROL ARE RECOMMENDABLE- because MDI could deliver drugs without patient coordination of aerosal delivery and they could penetrate more deeply into lungs for better results
IN THE CASE OF 18 YEAR OLD ASTHMATIC- If the patient is sensitive to propelet in metered dose inhaler -try dry powder delivery systems because they donot want to use propellants and patients find them easier to use correctly than meterd dose inhalers
