1. The GPTAL score is an acronym used by the doctors, nurses and other health care provider to made an outline of the obstetrical history of a woman. G stands for no. Pregnency, P stands for no. of preterm birth( baby born upto 37 weeks), T stands for no. of Term birth (baby born after 38 weeks), A stands for no. of Abortions or miscarriage and L stands for no. of living child.
I. In first case G3,T1, P0, A1, L1. As she is at present pregnent and alrady has a baby who was born in pre term.
ii. In second case of Anna the score will be G3, T1, P1, A1, L2. As the no. of pregnancy is same and the child is born in 33 weeks so, it is a preterm birth.
iii. In case of Brooke G3, T1, P0, A1, L1. As she is pregnant at present, and already gave birth of a girl at 41 weeks and had a miscarriage.
iv. In case of Alyssa G3, T2, P1, A0, L3. As she has 3 living children, no miscarriage and two were in term birth 39 1/7 weeks and 40 2/7 weeks and one was preterm at 37 weeks.
2. The most concern finding is the fundal hight is 19 cm.
As after 24 weeks of gestation the fundal hight is more or less 2 cm of the geastational age. But here it only 19 cm so, the growth of the fetas is slow and it requires to report to the physician.
3. It is unable to determine the EBD.
As Malia, 37 yrs. age woman is using YAZ birth control pills for last 3 yrs. in a regular basis there is no chance of getting pregnant. The withdrawal bleed is not a menstrual bleeding as it not contain the secondary oocyte and contain endothelial cells, hormones and some amount of blood.
4. The women may have placenta previa.
As she already has 2 C sections in previous 2 deliveries. And she had a sudden onset of intense localised pain in abdomen and had no bleeding it means the placenta may be blocked the cervix and due to previous C sections.
