You are the Director of Clinical Informatics at an academic medical center in the western United States. Part of your role is to oversee the patient portal which is considered part of the outpatient EHR. The center is planning to adopt and integrate a PHR with its EHR. The hospital CEO drafts of a vision statement that states, “By using the latest technology, University Hospital will improve how our patients experience Health Care. Instead of patients coming to us for help, we will be there whenever and wherever they need us, asking, ‘How can we help you?’ This initiative will make healthcare easier to access and more convenient to use, improve patients’ health, and reduce the rising cost of health care in our area.”

1. As a director of clinical informatics it is responsibility to keep maintain an optimal level of health of community people for there good health and wellbeing. So, my role will be =

a) Provide health education = the nurse's main role in community settings is to provide proper health education to the community people about their basic hygiene, common diseases that occurs the community and there prevention.

b) Use of proper resources = the resources of the health facilities available in the are must be used properly on the community people.

c) collaborative care = the another most important role is to collaborate with the other co- organizations like as politicians, local transport system, NGOs etc.

d) provide guidance to the community health care providers = it is another main duty is to provide guidance to the health care stuffs like as how they will educate the people, involve them in ther own care and various community health programs etc.

2. The patient has to ba provide and offline application includes proper details of correct Identificationcation data and the mistakes that has to correct. The stuffs of the office will define the problemand the analyse it properly. Then they will correct the data in main information file with maintaining confidentiality of the client. At last the information has to provide to the client about the correction of their data.

3. The barriers that ha to face to the PHR are mainly =

- lack of awareness of the servises about handling the hard-disk.

- lack of confidentiality is present there.

- various types of safety and security issues are there as the information can be hacked.

- inadequate internate service can hamper the whole process . Various other things are 4he barriers of the PHR.

We can over come those barriers by various way -

- trained employees has to accept in duty.

- all the stuffs has to provide effective education about why confidentiality maintain is so important and provide them direction how they will handle the data.

- proper software security has to maintain so that information can not be hacked.

- proper and uninterrupted internate service has to maintain. This responsibility has to take by the authorities and various other departments.
