Case Study 3: Jennifer (Teen mother/self-concept) Jennifer, is 17 years of age, she brings her 3-month old infant to the clinic for a routine check-up and immunizations. They live 1 hour away from the clinic in a small community with no public transit. The teen mother has dropped out of high school, having completed up to grade 10. She is accompanied by her boyfriend (not the biological father) and the two of them are arguing loudly in the waiting room. The baby is crying and very irritable, sitting in her car seat. There is no attempt to comfort the infant by either caregiver. As you approach the family, you notice a strong odour of cigarette smoke and urine. The infant is wearing clothing that is soiled with dried vomit. The mother states her baby has been drooling and spitting up a lot lately. Mom states she has been breastfeeding because it is “cheaper”, and now uses formula and rice cereal at bedtime to help the baby sleep. The infants weight today is the same as her 2-month check-up. Mom states the baby has not yet smiled or attempted to roll over. Furthermore, the client confides in you that her biggest concerns right now are how she feels about herself and how she looks.
1. Based on your understanding of the social determinants of health, identify two determinants that may negatively impact this infant’s health and explain why?
2. While providing nursing care to this infant refer to Erikson’s theory and Freud’s stages of development to help you understand the developmental stage of the child. Explain the developmental stage and how this stage influences your approach to the care you will provide.
3. Identify two stressors for this teen mother with altered self-concept and the rationale for the identified stressor.
4. Taking into account the development needs of this adolescent, how will you collaborate with her to establish priority interventions to address her self-concept difficulties.
1. determinants which affecting on infant's health
EDUCATION: the determinants which mostly affect this infant's health because her teen mother was dropped out from school since tenth standard so may she have not enough knowledge how to take care of child in healthy manner.
what nutrition infant required for proper growth and development.
SOCIAL AND COMMUNITY CONTEXT: this is one another determinant which can be affect to infant's health.
reason: she is living with her by friend not to with her husband according to history, there is no any family support to her , so nobody is taking proper care of this baby , also nobody is there to educate this mother how to take care for self and baby. which can may affect infant;s growth and development.
2. according to erikson's stages in infancy there is trust vs. mistrust which is mostly seen in infancy . because infant may be not trust everybody who is caring him but more you give hope that infant become more comfortable while taking care
fraud's stages of development are: oral stage , anal stage, phallic, latent phase , genital phase.
oral phase: mouth sucking, swellowing
anal phase: get control on withholding and expelling feaces
phallic phase: identify gender, role model male/female
latent phase: cognitive and social developments with peer group
genital phase: sexual and intimate development, individual attract to opposite gender
by understanding these are the phases of development it comes to know what are the elements and behaviour which can be develop in infancy.

Stressors : ROLE PERFORMANCE/ROLE CONFLICT,it can be affect her self concept , may she feel that she is not playing proper role as a mother while taking care to her infant which can be affect her self concept.
LOW SELF ESTEEM:, self esteem is state of mind where some of point she may not feeling that she is living healthy life and she is giving proper care to her child.
4.priority of intervention:
give knowledge where she need to change while taking her and her infant's care .
how to be positive about her life, where she have to focus to improve selfesteem.
