What is a patient-centered medical home?

Answer: The patient-centered medical home (PCMH) is a type of healthcare delivery model in which primary care is provided by a chosen physician who coordinates with a cooperative team of healthcare professionals to provide the necessary care when and where they need it. It takes collective responsibility for the comprehensive integrated care plan provided to the patient and maintaining the physical, mental and social health of the patient, building a strong physician-patient relationship.

What is an accountable care organization?

Answer:

It is a type of healthcare organization that coordinates with the group of healthcare professionals,hospitals, health agencies, pharmacists to provide high-quality care to the patient, and a reduction in the cost of care.

How they are different?

Answer:

Both differ in their payment mode. Person-centered medical home care providers are accountable themselves for providing services like tests and procedures.PCHM follows the traditional fee-for-service program i.e patient needs to pay differently for each test and procedure. But ACO organization is accountable to patients and third-party payers for the quality, appropriateness, and efficiency of the health care provided. Medicare is an ACO's primary payer. Other payers include private insurances and employer-purchased insurance. Members of ACOs are rewarded or penalized more based on their performance, for keeping their patients healthy and out of the hospital.
