Carl is a 65 year old male who manages an independent grocery store. Carl works very long days, arriving at the store at 5:30am most days and not leaving until 8pm. Carl is very hands on in the business and regularly unloads deliveries and stocks the shelves. Carl has early stages of heart disease that is manageable with medication. Carl is an ex-smoker, having smoked from the age of 18 – 53. Carl is a non-drinker and maintains a healthy diet. Carl’s wife has bought him a voucher for your clinic for his birthday as she felt that he needed to relax and unwind more, and he has reluctantly come along today to use it.

Carl may be stressed.Prolonged cigarette smoking has influence on glucose intolerance and low serum levels of high-density lipoprotein cholesterol (HDL).Cigarette smoking also is a cause of peripheral arterial disease , aortic aneurysm, coronary heart diseases and cerebrovascular disease.He may be at risk for even myocardial infarction.Smoking affects the respiratory system include: irritation of the trachea and larynx , reduced and retarded lung function ,breathlessness due to swelling and narrowing of the lung airways and excess mucus in the lung passages,permanent damage to the air sacs of the lungs and also reduces the amount of oxygen in the body's tissues and increases risk for blood clots.

He may have increased heart rate and blood pressure ,stickier blood because of carbon monoxide level increase,damage to the lining of the arteries, which is a major contributing factor to atherosclerosis.

2. A broad physical examination is done along with complete blood count, Chest Xray, spirometry,saliva test for nicotine, CT scan,electrocardiogram,ECHO,Hemoglobin A1c (HbA1c).

3.Some questions to ask him are

How did you start smoking? What functions or purpose does smoking play in your life? Have you tried to quit? Have you had respiratory problems?Current number of cigarettes per day?

Recommend stop smoking medication,refer him to individual or group counseling,Inform family and friends to support him to quit smoking.
