The child enters the emergency room with a head injury and a Glasgow Coma Score of 14 or more, and the child is not vomiting. It has been asked that the physician not order a CT scan. To ensure that all physicians are in compliance with the practice, a report will need to be generated for the chief marketing officers at each facility. To generate the report, you are asked to create a data collection plan. Include the following criteria into your plan:

Five W of data collection-

1. What data to collect
2. Form whom data is to be collected
3. Who will collect data
4. From where the data will be collected
5. When is the data to be collected

The nuber of data to be collected will depend on type of study,hypothesis to be tested,number of variables, types of statical computetin, in and expirimental study like this number will be also be determined by the number of expirimental annd control groups. One needs also to consider the method of administration the nature of treatment and the schedule.

Criteria of good reporting,

• Can be made promptly
• Clear concise and complete
• All pertinent, identifiying data included
• Mention people concerned situations and signatur of person making report
• Easily understood
• Important points are emphasized.
