• What are the health practices followed by your family to improve health?
• What is the family goal towards health & ways to achieve it?

2) Nutrition:

• How many meals you have daily?
• What is your average calorie intake daily?
• What type of diet do you prefer?

3) Sleep/Rest:

• How often you sleep daily?
• What do you think at an average how many hours of sleep per day is required to stay healthy?
• Does any of your family members has sleep disturbances/ problems?

4) Elimination:

• How often you have bowel movements?
• How often you urinate?
• Does any of your family members face any problems related to bowel or Urinary elimination?

5) Activity/Exercise:

• Do you perform exercise on regular basis/ explain your schedule of activity.
• Describe health benefits of exercise.
• What is the best time for exercise?
