People with acute care health problems should be given instant treatment in the hospital. So, there is a emergency department and doctors always present for the treatment of the patients. There are all the facilities for the treatment for a severe injury or episode of illness, an urgent medical condition, or during recovery from surgery. Apart from this many other important facilities are aslo being provided such as: Ambulatory Care Facility, Home Health Agency, End Stage Renal Disease Facility (dialysis center), Hospice etc.
For the acute pateint with a disability there are wheelchairs, so that they may not have to walk or take the stress, always an attendee is available for the particular person. And also services and facilities related to parking area, reception, and appointment, elevator, and physiotherapy available for instant work.
If we talk about the patient with complex healthcare needs, they are the most important who has to be look after and given more concern. Patients who have complex health needs require both medical and social services and support from a wide variety of providers and caregivers, and the patient-centered medical home (PCMH) offers a promising model for providing comprehensive, coordinated care. Smaller practices, however, face particular challenges in coordinating care for these patients.
