Cindy, 13, with poor nutritional intake, Bulimia Nervosa. Recently lost his father to alcoholism. lives with her mother in a private home. Has insurance coverage through mother. Resides in New York, Brooklyn, Zip code 11205
A client scheduled for discharge back to their community New York, Brooklyn, Zip code 11205 from the acute care setting. As the Community Health Nurse assigned to be the Case Manager for this client, you will be required to prepare a discharge plan of care for the client (template provided). Plan of care must focus on Primary, Secondary and Tertiary levels of prevention for management of the client while in the community, resources and knowledge of the resources available within the community. Students provided with a discharge plan template.

Bulimia Nervousa is an eating disorder which includes binge eating followed by humiliation,guilt and self-depression.
SMART OBJECTIVE: To assess the health condition of the client andplan care specfically on her health, with measurable actions tha is achievable , and help the patient having a feeling of self esteem within a partucular time.
Nursing diagnosis:1.Imbalanced nutrition less than body requirements
Primary prevention needs:Assess the health status of the client and teach the family members to about the importance of her health maintenance.
Secondary prevention needs: 1.Set a meal time for each meal.
2. Develop a healthy meal plan consulting with dietitian.
3. plan a structured 3-5 meals a day to reduce binge eating.

5. Note in a diary regarding thetype of food they consumed.
Territiary prevention needs:Ineffective coping with the environment and family members
