Discuss why Enteral Nutrition is recommended over Parental Nutrion in critically ill patients who are hemodynamically stable and have a functional gastrointestical (GI) tract.
ABSTRACT
Enternal nutrition is the most effective method of administration of nutrition than parental nutrition and has least complications comparing to parental nutrition and more effective in critical ill patients who cannot attain an adequate oral intake from food and/or oral nutritional supplements, or who cannot eat/drink safely. The aim is to improve nutritional intake and so improve or maintain nutritional status.
INTRODUCTION
Enternal nutrition is defined as administration of required nutrients to the person through NG tube or, by gastrostomy or jejunostomy, This type of feed is mainly given in Patients who either are unable to take any nutrition orally or who are unable to take sufficient nutrition orally, but in whom the gastrointestinal tract is functioning, may be fed enterally. It is less expensive than parenteral nutrition and is preferred in most cases because of less severe complications and better patient outcomes, including infections, and hospital cost and length of stay.
BODY
Critical ill patients typically experience catabolic stress and systemic inflammatory response; in turn, these responses alter both the morphology and function of the gastrointestinal tract. ICU patients suffer GI dysfunction due to impaired GI motility, digestion, or absorption and also due to inadequate caloric intact can make malnourishment the patients .so it is essential to give adequate nutrition. In addition, enteral feed is less expensive than parental nutrition and also has less chance of complications than that of parental nutrition. Furthermore, enternal feed stimulate intestinal growth and function, both directly intraluminally, because it supplies substrates for enterocyte oxidation, and indirectly, because it promotes hormone secretion through the intestinal trophic effect, which would reduce bacterial translocation and the problems associated with it. Finally, enternal nutrition can be given to only critical patient who is haemodynamically stable. If patient is unstable parental nutrition is the only route.
CONCLUSION
Enternal nutrition is administration of essential nutrients through gastrointestinal tract through tube. They are given to patients who are unable to take oral feed. And mainly critical ill patients who haemodynamically stable are given enteral nutrition.
