History collection is the most important process which helps in diagnosis of a disease condition in a patient.
a) Past history:
This includes all the past medical (any disease in medication) and surgical history ( operations underwent)
Past occupational history helps us to identify the type of work the patient was doing and whether the current health status or illness as resulted out of its ill effects.
b) Social history :
This includes the collection of the educational status, occupation of the client,any disabilities physically, Substance or drug abuse, Marital status if married details of spouse to be shared, recreational activities
c) Review of the system
These are list of questions asked by a Doctor to patients.
Asking questions related to each organ enables to provide the most appropriate data needed for diagnosis and treatment
d) Family history
This describes about a patient whether he is a adopted child or not.parents , grand parents disease condition is asked for getting to know any hereditary diseases, Sibling health status could help us to know any particular illnesses.
